Services:- Consumer-Directed Personal Assistance
- Adult day health care
- Personal care services
- Personal Emergency Response systems (PERS) (only for people age 14 and older)
- Respite care (720 hours maximum a year)
Eligibility: Must meet nursing home criteria,income limits.
Intake: Contact the local Department of Social Services and ask for a Nursing Home Pre-Admission screening if the individual is in the community. If the individual is in the hospital, contact the discharge planner.
